AlphaCare+ is a home health agency based in Swansea, Massachusetts. Our nurses, therapists and aides bring skilled clinical care into people's homes across southeastern Massachusetts — so recovery, chronic illness and daily support do not have to mean leaving home.

CHAP accredited

AlphaCare+ is accredited by the Community Health Accreditation Partner, an independent body that reviews home health agencies against national standards of care, safety and management.

Licensed Massachusetts clinicians

Every nurse, therapist and aide who enters a home is credentialed and licensed to practice in Massachusetts, with credentials verified before their first visit and monitored after it.

Quality assurance on every case

A dedicated Quality Assurance Manager reviews documentation and outcomes, and clinical field supervisors observe care in the home rather than reviewing it only on paper.

Active in the Home Care Alliance of Massachusetts

AlphaCare is an active participant in the Home Care Alliance of Massachusetts, the state association for home care and home health providers — which keeps us current on regulation, payer policy and practice standards as they change.

Private homes

Apartments, houses and family homes — the majority of our visits.

Assisted living facilities

Skilled nursing and therapy for residents, coordinated with facility staff rather than duplicating them.

Group homes

Clinical support for residents with complex medical and behavioral needs, delivered on site.

Sober homes

Nursing support for residents in recovery, including medication oversight and monitoring.

Foster families

We work with foster families caring for members with medical or behavioral health needs.

Subsidized housing

Skilled nursing, therapy and aide services for residents of subsidized and senior housing developments.

What we are known for

Medication management

Medication is where home health cases most often go wrong — missed doses, duplicated prescriptions, changes after a hospital stay that never reach the person taking the pills. It is the part of our work we have built the most depth in, particularly for the behavioral health population, where regimens are complex, adherence is fragile and the consequences of getting it wrong show up fast. Our clinical leadership includes a psychiatric mental health nurse practitioner, and our nurses set up, reconcile and monitor medication regimens in coordination with prescribers and pharmacies.
